“Brain fog” and “boost your memory” are among the most searched health phrases on the internet, and among the most heavily monetised. Around them sits an industry of supplements, nootropics, and apps promising to sharpen a dull mind. The honest answer to what actually works is quieter and much harder to sell, and it starts by taking the phrase apart.
This is a reading of the evidence, not medical advice. If brain fog is persistent or getting worse, the right first step is a doctor, for reasons that will become clear.
Because the useful thing to understand is that brain fog is not a diagnosis. It is a lay term for a subjective experience: trouble concentrating, a slower recall of words and names, a sense of mental heaviness. Symptoms like that have causes, and the causes are where any real fix has to begin.
A symptom, not a condition
The list of things that commonly produce that foggy feeling is unglamorous and long. Poor or insufficient sleep is the most common by far. Chronic stress does it, as does dehydration and the side effects of ordinary medications. So do a range of medical conditions that have nothing to do with the brain being “tired”: an underactive thyroid, anaemia or low vitamin B12, the hormonal shifts of perimenopause, depression and anxiety, and post-viral states, including the cognitive symptoms many people report after COVID-19.
That variety is the whole point. The question “how do I fix brain fog” has no general answer, because the fog is a signal, not a thing in itself. Two people with identical symptoms might need a sleep change and a blood test respectively. Neither needs the same pill.
The pill that was supposed to work
Which brings us to the supplements, because they are what most people reach for first. The best-known memory aid is ginkgo biloba, sold for decades on the promise of sharper recall. It has also been tested more rigorously than almost any rival, and the result is worth knowing.
In the Ginkgo Evaluation of Memory study, published in the Journal of the American Medical Association in 2008, researchers led by Steven DeKosky followed more than 3,000 older adults for around six years, randomly assigning them to take ginkgo or a placebo. The supplement did not reduce the rate of dementia, and a companion analysis found it did not slow the ordinary decline of memory and thinking either. It remains the largest trial of its kind, and it is a clear null.
This does not test every product on every shelf, and absence of evidence is not always evidence of absence. But the most studied memory supplement, given the fairest possible test, did nothing. That is a strong prior to bring to the rest of the aisle, and a reason to treat “clinically shown to support memory” language with suspicion rather than hope.
What the evidence actually points to
When you strip out the products, what remains is a short list of dull interventions, and one of them dominates.
Sleep is the closest thing to a genuine lever. Losing it reliably degrades attention and working memory, the exact faculties that feel broken during a foggy day, and sleep does positive work too: it is when the brain consolidates the day’s learning into durable memory, a mechanism reviewed at length by the sleep researchers Susanne Diekelmann and Jan Born. A great deal of what people experience as failing memory is closer to unpaid sleep debt. Regular physical exercise has a smaller but real association with better cognition, and beyond that the gains come from treating whatever underlying cause the fog is pointing at.
Notice the shape of this. Almost none of it “boosts” a healthy memory beyond its normal ceiling. It removes the things dragging it below that ceiling. The honest verb is not boost but unblock.
Why the market prefers addition
The deeper reason the useful answer stays buried is commercial. A pill is a product; a night of proper sleep and a trip to the doctor are not. The industry sells addition, the idea that you can bolt extra capacity onto a working brain, because subtraction, removing the sleep loss or the untreated condition that is causing the problem, cannot be packaged. The evidence keeps pointing at subtraction anyway.
A few caveats belong here. Individual studies are not the same as settled bodies of evidence, effects vary between people, and none of this is a claim that every supplement is worthless or that a sharp memory is purely a matter of habits. The point is narrower: the specific promise of fixing fog and boosting memory with a purchase is not well supported, and the things that are supported are mostly free.
It is worth repeating the one genuinely important line. Because persistent brain fog can be an early sign of something treatable, from a thyroid problem to a sleep disorder to depression, the most useful response is not a supplement but a conversation with a doctor who can look for a cause.
The disappointment in all of this is also the good news. There is probably no fog-lifting compound waiting to be discovered in a capsule, but there is often a real and fixable reason behind the fog, and finding it does more than any nootropic on the market.
