A man sits across from a neurologist in Iowa in the late 1980s. He is intelligent, articulate, socially fluent. Asked to pick between two appointment dates, he spends nearly half an hour weighing every conceivable consequence of each — weather, traffic, the neurologist’s schedule, potential conflicts on his calendar — and arrives at no decision. His IQ is intact. His memory is fine. What he has lost, after surgery to remove a brain tumour, is the small tug in the chest that most people call a hunch.
That patient, and others like him, became the empirical spine of one of the strangest arguments in modern neuroscience: that the body decides first, and the reasoning brain follows.
The neurologist who watched rational people stop being able to choose
Antonio Damasio, a neurologist working at the University of Iowa and later the University of Southern California, worked with patients who had suffered lesions to the ventromedial prefrontal cortex — a small patch of tissue sitting just above the eye sockets. On every conventional test of intelligence, these patients performed normally. On the test of actually running a life, they collapsed.
They lost jobs. They ruined marriages. They made catastrophic financial decisions and then, given the same choice again, made them a second time. What linked their failures was not a loss of information. It was a loss of feeling.
Damasio’s insight, published in his book Descartes’ Error and refined across experiments with the neuroscientist Antoine Bechara, was that emotion is not the enemy of good decision-making. It is the mechanism that makes decision-making possible at all. He called the signals that guide us somatic markers — bodily traces of past experience that flag an option as promising or dangerous before conscious reasoning has begun.
The card game that broke the theory of the rational agent
To test the idea, Bechara and Damasio designed what became known as the Iowa Gambling Task. Participants sit in front of four decks of cards. Two decks pay large rewards but occasionally deliver ruinous losses. Two pay smaller rewards but are, over time, profitable. There is no rule sheet. Players have to figure out the system by playing.
Healthy participants start by sampling all four decks. Early in the game, something happens that they cannot yet articulate: their skin conductance rises when their hand hovers over a losing pile. The body is registering the pattern before the mind has language for it. After repeated exposure, most healthy players develop a hunch. Eventually, they can explain why.
Patients with damage to the ventromedial prefrontal cortex never generate the skin conductance response. And they never learn. They keep drawing from the bad decks even after being told, in plain English, which decks are bad. Knowing was not enough. The body had to know too.
Why a functioning IQ is not a functioning life
One of Damasio’s patients was known in the literature as Elliot. An accountant before a tumour was removed from his frontal lobe, Elliot emerged from surgery with his intellect intact. He scored in the superior range on every cognitive test the Iowa team gave him. He could reason about ethics, forecast consequences, and describe in exhaustive detail what a reasonable person ought to do in any given situation.
He could not act on any of it. Elliot lost his job for spending entire afternoons deciding how to file a single document. He entered into business partnerships that anyone else would have refused within five minutes. He divorced, remarried a woman his family warned him against, divorced again. When Damasio asked him how he felt about the wreckage of his life, Elliot described it with the emotional register of someone reading a stranger’s obituary.
The reasoning machine was still running. The signal that tells you which of a thousand possible futures is worth walking toward had gone silent.
The body as a decision-making organ
Damasio’s framework proposes that every meaningful experience leaves a physiological trace — a tightening in the chest, a churn in the stomach, a lift in the shoulders — and that the brain files these bodily states alongside the situations that produced them. When a similar situation arises later, the body replays a compressed version of the old sensation. That replay is the somatic marker. It narrows the field before conscious deliberation begins.
Without it, every option looks equally weighted. Every path forward is a paragraph of pros and cons with no way to break the tie. This is why Damasio’s patients could reason indefinitely and choose nothing. They had the ability to weigh. They had lost the scale.
Follow-up work has extended the theory into the study of the insula, a deeper brain region that maps the internal state of the body — heart rate, gut sensation, breath — and feeds that information into decision-making circuits. Research on the insula suggests that anxiety, addiction and impulsive choice all involve disruption to the same body-to-brain signalling loop Damasio first mapped.
What addiction reveals about the machinery in reverse
If Damasio’s patients lost the ability to feel their way toward good decisions, people with substance dependence show what happens when the somatic marker system is hijacked rather than silenced. The somatic marker framework applied to addiction suggests that chronic drug use rewires the bodily signals that ought to warn against a harmful choice. The chest tightening that used to say stop starts saying go.
Building on this understanding, the insula has been identified as a candidate target for brain stimulation treatments in addiction — precisely because interrupting the faulty body-to-brain signal appears to loosen the grip of craving. The circuit Damasio described in patients who could not decide is the same circuit that, in a different failure mode, decides too fast and too often for the wrong thing.
Why the theory keeps mattering outside the clinic
The somatic marker hypothesis has never had a quiet life. The idea has been invoked to explain why expert traders sometimes outperform purely statistical models, why people choose products that feel right over ones with better specifications, and why experienced decision-makers often sense patterns before they can articulate them.
Critics have pushed back on parts of the Iowa Gambling Task methodology and questioned whether skin conductance really captures what Damasio thinks it captures. The debate is live. What has held up is the core clinical observation: strip out the emotional signal, and reasoning stops producing choices.
That observation has an uncomfortable implication for a culture that treats deliberation as the highest form of intelligence. The people who cannot decide are not the ones ruled by feeling. They are the ones whose feelings have been surgically or chemically muted. The body often knows things the conscious mind cannot yet name — a pattern of tension, a mismatch between someone’s words and their face, a shift in a room that arrives as sensation before it arrives as thought.
The quiet argument buried in the data
What Damasio’s patients demonstrated, in the most literal way possible, is that the folk distinction between trusting your gut and thinking it through is a false binary. The gut is part of the thinking. It is the compression layer, the filter that reduces an impossible search space to a manageable one. Without it, deliberation runs forever and terminates in nothing.
The reason this matters for anyone reading it in an ordinary body, with an intact prefrontal cortex, is that the somatic marker system can be dulled without any surgery at all. Chronic stress, dissociation, sleep deprivation and long stretches of overriding physical signals in the service of productivity all reduce the fidelity of the body’s messages. The organ that Damasio’s patients lost to a scalpel, the rest of us can quietly train ourselves to ignore.
The clinical literature suggests that when the signal goes, so does the ability to choose. What remains is what Elliot had — a functioning intellect, a wrecked life, and no felt sense of which direction to walk in next.
The reasoning brain, it turns out, was never the pilot. It was the co-pilot all along.
